1. In order to complete a home equity application you will need the following information:
   • Social Security Number
   • Mortgage information
   • Estimated home value
   • Property ownership information
   • Employer’s name and address
   • Asset balance information
     
     
2. You may complete the application using Acrobat Reader on your computer OR you may print it and complete is by hand. If you are filling this application out electronically and do not complete, save this PDF to your hard drive to be completed at a later time.
   
   
3. Return the completed application to United Republic Bank by submitting online (click submit at the end of the application), fax or mail. Once we receive your application, we will send you an e-mail alert to let you know that we have received it and we will send you any applicable information within three business days.
